Most OB/GYN billing questions don't need a billing company — they need a clear, accurate answer. Which taxonomy code should a new maternal-fetal medicine physician use? How do you sequence two O codes on the same claim? What replaces 59400 in 2027? Is there a CPT code for robotic hysterectomy?
We answer questions like these every week for our clients, so we've turned the most common ones into free reference guides. They're written by our billing team, based on more than eight years of OB/GYN billing and coding experience, and updated when the rules change. Use them to train new staff, settle a coding question or prepare for a payer conversation.
How to use these guides
For practice managers and billers
Bookmark the CPT code library and the ICD-10 O codes guide. Together they cover the codes and rules behind most OB/GYN claims. When a claim denies, compare it against the relevant section before resubmitting — many CO-11 and CO-16 denials trace back to diagnosis sequencing or missing companion codes like Z37 and Z3A.
For physicians and practice owners
Start with the 2027 maternity transition guide. The end of the global maternity package on January 1, 2027 is the largest change to obstetric billing in decades, and it affects documentation, cash flow, patient cost-sharing and payer contracts. If you're adding a provider or opening a practice, the taxonomy code guide explains the codes you'll need for enrollment.
For anyone evaluating their billing
Try the revenue leakage calculator for a two-minute estimate of your net collection rate and the cost of denials. It's not a substitute for an audit, but it's a good way to decide whether billing deserves a closer look.
Our weekly blog
Our blog publishes a new OB/GYN billing article every week, focused on the questions practices actually ask: how to fix specific denial codes, how to code common procedures, how payers handle split maternity care, how to use telehealth modifiers, and how to prepare for regulatory changes. Each article links to the related service and resource pages so you can go deeper.
A note on accuracy
Coding rules change. CPT codes are updated every January, ICD-10-CM every October, and payer policies whenever a payer decides. We date our resource pages and update them when rules change, but you should always confirm codes against the current CPT codebook, the current ICD-10-CM Official Guidelines, and your payers' published policies before relying on any reference — including ours.
These guides are educational. They aren't legal advice, and they don't replace payer-specific guidance or the judgment of a certified coder reviewing the actual documentation for a specific encounter.
What's in each guide
2027 maternity code transition
On January 1, 2027, the CPT code set deletes the global maternity package — 59400, 59510, 59610, 59618 and the related delivery-only, antepartum and postpartum codes. Prenatal and postpartum visits move to E/M reporting, and new codes report labor management and delivery. The guide covers what's deleted, what's new, how each phase of care will be reported, what it means for cash flow, documentation, patient cost-sharing and payer contracts, and a readiness checklist. Read the 2027 guide.
ICD-10 O codes and sequencing
Chapter 15 of ICD-10-CM has its own sequencing rules, trimester characters and fetus identifiers. The guide explains when O codes come first, how to choose between Z34 and O09 for prenatal visits, which companion Z codes are required on delivery claims, when O80 applies, and the most common O-code errors on OB claims. Read the O codes guide.
OB/GYN taxonomy codes
Every provider's NPI record lists a taxonomy code that payers use to match claims to enrollment. The guide lists 207V00000X and every OB/GYN subspecialty taxonomy, the codes for midwives and women's health nurse practitioners, how taxonomy appears on claims, and how to update it without causing rejections. Read the taxonomy guide.
OB/GYN CPT code library
A quick reference to the codes used most in women's health — obstetric care and procedures, obstetric and gynecologic ultrasound, preventive visits, contraception and LARC, office procedures, hysteroscopy, laparoscopy, hysterectomy and pelvic floor surgery — with notes on the most common pitfalls. Open the CPT library.
How our guides are researched
Each guide is written by our billing team and based on primary sources wherever possible: the CPT codebook and its guidelines, the ICD-10-CM Official Guidelines for Coding and Reporting, CMS's Medicare Physician Fee Schedule and NCCI materials, NUCC's taxonomy code set, AMA announcements about the 2027 maternity changes, and payers' published policies. When rules are still being finalized — as with parts of the 2027 transition — we say so, and we update the guide when final guidance is released.
