Quick-reference coding guide

OB/GYN CPT Code Library

The CPT codes OB/GYN practices use most — obstetric care, ultrasound, contraception, office procedures and gynecologic surgery — organized in one reference, with notes on the most common billing pitfalls and the January 2027 maternity code changes.

How to use this library

This reference lists the CPT and HCPCS codes we see most often in OB/GYN billing, grouped by type of service. Descriptors are summarized for readability — always confirm the full descriptor in the current CPT codebook, and check payer policies for coverage. Codes are valid for 2026 dates of service unless noted.

Frequently asked questions

What are the most common OB/GYN CPT codes?

Office E/M codes (99202–99215), preventive visit codes (99384–99387, 99394–99397), obstetric ultrasound (76801–76817), global obstetric codes such as 59400 and 59510 (through 2026), LARC codes 58300 and 11981, endometrial biopsy 58100, colposcopy 57452–57456, and hysteroscopy and laparoscopy codes such as 58558, 58661 and 58571.

What is CPT 58552?

CPT 58552 is laparoscopy, surgical, with vaginal hysterectomy, for uterus 250 g or less, with removal of tube(s) and/or ovary(s) — a laparoscopically assisted vaginal hysterectomy (LAVH) with adnexal removal. For a uterus over 250 g with adnexal removal, use 58554.

What is CPT 58940?

CPT 58940 is oophorectomy, partial or total, unilateral or bilateral — an open (abdominal) approach. Laparoscopic removal of the ovary and/or tube is reported with 58661.

What is the CPT code for IUD removal?

Removal of an intrauterine device is CPT 58301. Insertion is 58300, and the device itself is reported with the appropriate HCPCS J-code. Laparoscopic retrieval of a migrated IUD has no specific code and is usually reported with an unlisted laparoscopy code per payer guidance.

Are obstetric CPT codes changing in 2027?

Yes. The global maternity codes and related delivery and component codes are deleted for dates of service on or after January 1, 2027, and new labor management and delivery codes are added. See our 2027 maternity transition guide.
